Demographic (city, state, zip, phone, etc) fields and filter by city and state mod

I've created a mod the adds fields to the link submittal process. The fields are city, state, zip, phone number, and cell number. In addition, this mod also allows the user to filter results by state and city once they have clicked on a category.
