open submit.tpl and find
Content visible to registered users only.now if you wish you can replace this or just add a Content visible to registered users only. after it and add this code

*note*in the above code make sure to edit <b>Name:</b> YOUR NAME HERE<br /><b>Email:</b> YOUR EMAIL HERE at the end of the code with your information :)
