New Category Overwrites?

I've been trying out the latest version in hopes of transferring an existing directory. One issue is puzzling me:

When adding new categories, the app overwrites an existing (random?) category. I've tried this in both FF and IE with the same result. I can add categories manually using phpAdmin or ODBC.

Any ideas of what could be wrong?

I've recently tried this it on one of my existing servers, thinking maybe my staging server was causing the issue. Still the same results. Has anyone run into this before?
