installation problems

hi there!
I'm trying to install phpLD 2 on my server but without success.
When I visit mysite.com/install/index.php

Requirements

PHP Version >= 4.1 (4.3.11)
Server API (apache)
GD Support (for visual confirmations) (2.0.28)
Session Save Path writable? (/tmp)
MSSQL Server Support Not available.
MySQL Support (4.0.24-standard)
PostgreSQL Support Not available.
./include/config.php writable? OK
./temp writable? OK
./temp/templates writable? OK

now, where's the problem?

did you make sure to:

CHMOD 777

include/config.php
temp/


Create Folders and CHMOD 777 them:
temp/adodb
temp/cache
temp/templates

Everything else looks fine from what you posted.

just done!
now I've problems with Database Server. I've included localhost, 72.29.76.206 (IP adress), sh74.surpasshosting.com, but they don't work!! :?

ftp access
host?
user?/pswd?

db
user?/pswd?
table?

if no success, then cpanel acccess
