Contact Page Help!

Hey,all.
I am using phpld 3.06 on my directory (http://www.directoryforyou.com)
and using the "Stone Flash"template..
But it seems that "Contact Us" page doesn't work
should i create one by myself?
and how?
I am a newbie and don't know too much about PHP
any help will be highly appreciated.
